Do you need to obtain a copy of your medical records from our office? Do you need our office to request your medical records from another doctor's office? Click here to print a copy of our Medical Records Release form. Simply complete and return to our office via fax at (913) 677-4514 or via email at . Please allow 7-10 business days for processing.

Patient Forms

The following forms can be completed prior to your visit to help save you time and make your check-in process easier. Feel free to contact our office if you have any questions!

  • Please make sure you have an appointment scheduled with our office prior to completing any online forms. This will help avoid processing errors.
  • Make sure to complete all forms under the applicable section, and notice that some forms will be completed online, while others will be printed for you to complete and bring to your appointment.
  • Please know that unfortunately we are not able to load all of our forms online, and there may be some additional forms / verification of information that you will need to complete when you are in the office.
  • Don't have a printer? No worries! Just notify our front desk, and we can supply you with a copy to complete at the time of your visit. However, to expedite the process, we do recommend that you review the form prior to your visit.
  • Please know that all information is completely confidential and is transmitted and maintained on a secure system.
